This event is for current Collins students only, and your student must be in attendance at school November 14th to participate in the event. Payments are nonrefundable.
Students will stay after school in their LC until after dismissal. Then the fun begins!
Pick up will be 7 PM SHARP. 5th graders will be picked up via carline in the front of the school and 6th graders will be picked up via carline at the back of the school. You will need you student(s)' car rider sign displayed in your vehicle's window. Cars picking up students from both grade levels will be required to pickup from the back of the school; please inform all students of where you will be picking them up.

PARENT PERMISSION AND RELEASE: By purchasing a registration, I agree to allow my child to participate in the PTO sponsored event Fall Frenzy. I understand that while student safety is a high priority for the District, under state law, the District is not responsible for medical costs associated with a student injury. I agree to release CISD, its Board of Trustees, employees, agents, and volunteers from any claims or causes of action, made against it or them on behalf of my child. I authorize and consent to immediate care and treatment for my child by any physician, nurse, hospital or District representative as a result of any injury or sickness my child may suffer related to this event. I voluntarily submit this registration with full understanding of its significance.
